Who is eligible for assistance?

	A child who has been diagnosed with a life-threatening medical condition
	The diagnosed child must be in treatment for a life-threatening medical condition
	The diagnosed child must be a resident of a local community served by The Let It Be Foundation
	Treating physicians must verify diagnosis in treatment and complete verification form

Who can refer a child for assistance?
Children may be referred to receive assistance by any of the following sources:

	Licensed medical professionals involved in the treatment of the child
	A parent or legal guardian
	A pastor/minister of the child and family
	The principal of the child's school

How does the approval process work?
The board of directors review all applications monthly. Notifications will be sent to applicants upon physician verification.
